No, the thesis is not clear to point out.  The thesis should be your purpose.  Why are you writing the paper?

3.     Reviewer question: Does the essay’s body stick to the main topic? If not, where does it digress, and how could the writer revise the paper to make it stay more on the main topic?

Yes, however, I do not think you have a good grasp on how this analysis was to be written.  The analyst was invited to assess what happened after it was over and help the company make sure it never happened again.  However, if I did not know this, you did stick to what the introduction paragraphs said.
