The health status of a community is the result of a myriad of factors impacting where people live, work and play. From public health policy to the built environment, community structures are integral to the decisions we make relative to health. This week, you will explore the some of the key indicators of health outcomes in communities, specifically, communities near you.
The Centers for Disease Control and Prevention (CDC) has developed Community Health Status Indicators or CHSI 2015. This is an interactive web application that produces health profiles for all 3,143 counties in the United States. According to the CDC (2015), “Each profile includes key indicators of health outcomes, which describe the population health status of a county and factors that have the potential to influence health outcomes, such as health care access and quality, health behaviors, social factors and the physical environment.”
Visit the CHSI 2015 webpage and perform a search for data on the county/ state combination closest to their residential location. If your county/state combination does not yield usable results, please choose another combination that is local to you. Once you have your location results, review the county demographics and summary comparison for all indicators.

After obtaining your location’s results, choose three (3) health indicators of greatest to you. Using the Community Health Status Indicators Template found in your Week 1 Resources, complete a summary of your community’s health status.
Per the template, your summary must include:
1. Demographic Overview
2. Description of three (3) indicators, including data reflecting populations most affected, the indicator’s relationship to Healthy People 2020 goals and associated indicators.
3. Discussion on possible factors contributing to these health issues in the community, including policy, build environment, or behavioral concerns.
Reference citation using APA 6th edition formatting is required.
